LODD for Miami Township Fire Department- March 20, 2001
Firefighter William Ellison III died of burns sustained in a residential fire on March 8, 2001. He was searching for a victim reported to be trapped in the burning house when the first floor collapsed, causing him to fall into the basement.

Firefighter Oscar Armstrong
LODD for Cincinnati Fire Department- March 21, 2003
Firefighter Oscar Armstrong was fatally injured in a flashover during a house fire. He was on the interior attack crew and had just gone through the front door of a single family residence when the flashover occurred.

Firefighter Daryl Gordon
LODD for Cincinnati Fire Department- March 26, 2015
Firefighter Daryl Gordon died after falling down an unsecured elevator shaft. He and his crew were assigned to search for occupants on the fifth floor of an apartment building when the event occurred.
